Secondary Bleeding after Hemorrhoidectomy

Abstract
The authors experienced 12 cases of secondary bleeding after 1,700 hemorrhoidectomies at Song Do Hospital form Jan. 1, 1991 to Dec. 31, 1991.
@ES We analyzed them and obtained the following results:
@EN 1) Total bleeding inidence was 0.7%, but it was reduced to 0.2% after a modification of the operation procedure(p<0.05).
2) bleeding days were between the 5th and 13th days, among which 10 cases98.3%) occurred between the 7th and 11th days.
3) As predisposing factors, 2 cases had suspected liver disease, among which 1 cases showed a low platelet count.
4) With regard to treatment, suture-ligation was performed in 4 cases, Foley catheter traction in 3, and conservative methods in 5.
5) The prevent bleeding, three points were emphasized during the operations in addition to the general principles:
¨ç suture-ligation of the feeding vessels underneath the mucosa to secure hemostasis,
¨è Protection of the internal sphincter from injury during dissection of the hemorrhoidal mass,
¨é Preservation of an intact area wider than 1 cm between the two hemorrhoidectomy sites.
